In a landmark achievement for both India’s healthcare and railway systems, Indian Railways has successfully transported the country’s first live donor heart by train, using the Mumbai Central–Gandhinagar Capital Vande Bharat Express to carry the organ from Surat to Ahmedabad.
The historic mission demonstrated how India’s expanding rail network can play a critical role in time-sensitive medical emergencies, giving a new lease on life to a critically ill heart patient.

Green Corridor Ensures Heart Reaches Hospital on Time
The donor heart travelled nearly 250 kilometres in about two hours, from Surat to the U.N. Mehta Institute of Cardiology and Research Centre in Ahmedabad. After the train arrived at Ahmedabad railway station, a specially created green corridor ensured the organ was transported without delay to the hospital, where transplant surgeons successfully implanted it into the recipient.
The complex operation required seamless coordination between Indian Railways, the Railway Protection Force (RPF), Gujarat Police, transplant coordinators, and medical teams. The donor heart was carefully transported to Surat railway station and placed aboard Train No. 20901 Mumbai Central–Gandhinagar Capital Vande Bharat Express, chosen for its speed, punctuality, and reliable schedule.

Indian Railways Opens a New Chapter in Emergency Healthcare
Once the train reached Ahmedabad, authorities had already established a dedicated green corridor from Platform 1 to the U.N. Mehta Institute, allowing the ambulance carrying the heart to bypass traffic and reach the hospital within the narrow time window required for transplantation. Every minute is critical during heart transplants, as the organ generally remains viable for only four to six hours after retrieval.
Railway officials described the operation as a proud milestone that showcased how the national rail network can support emergency healthcare beyond its traditional role of transporting passengers and freight.
Ahmedabad Divisional Railway Manager Ved Prakash said the mission reflected Indian Railways’ commitment to public service, adding that the coordinated efforts of railway staff, police, and healthcare professionals made the life-saving operation possible. Officials believe the success could pave the way for greater use of trains in transporting organs and other critical medical supplies over medium and long distances.
Unlike kidneys, which can often be preserved for many hours, a donor heart has one of the shortest preservation windows among transplant organs. Surgeons generally aim to transplant a heart within four to six hours of retrieval to maximize the chances of a successful outcome.
To meet these strict timelines, transplant teams often rely on “green corridors”—special traffic-free routes created by police to allow ambulances carrying donor organs to move quickly between hospitals. The successful use of the Vande Bharat Express adds a new option to India’s organ transport network, particularly when road travel over longer distances may be affected by congestion or weather.

NOTTO Drives Growth in Organ Transplants
The historic mission comes as India continues to expand its organ donation and transplant infrastructure. Under the National Organ and Tissue Transplant Organization (NOTTO), the country has seen a steady rise in organ transplants over the past decade, supported by improved coordination between hospitals, transplant registries, and state governments.
The achievement comes as India observes Indian Organ Donation Day on August 3 and prepares for World Organ Donation Day on August 13, both of which aim to encourage more people to pledge their organs and raise awareness about deceased organ donation. At the same time, states are increasingly using digital technology to strengthen transplant coordination.
In Tamil Nadu, the Transplant Authority of Tamil Nadu (TRANSTAN) has introduced the Vidiyal platform, which enables hospitals to register donors and recipients, manage waiting lists, coordinate organ allocation, and track transplants in real time, helping improve transparency and reduce delays. ()
Despite record transplant numbers in recent years, India continues to face a shortage of donor organs, with thousands of patients still waiting for life-saving transplants every year.
