The approval was supported by the phase 3 RASolute 302 trial, in which daraxonrasib significantly improved overall survival, progression-free survival, and tumor response compared with standard chemotherapy.

Daraxonrasib takes a broader approach.

It is an oral RAS(ON) multiselective inhibitor that targets the active, GTP-bound form of mutant and wild-type RAS. The study describes it as a tri-complex inhibitor.

The mechanism can be understood through its “molecular glue” approach:

• Daraxonrasib first binds to a helper protein called cyclophilin A.
• This creates a new binding surface.
• The resulting structure can interact with RAS.
• This interferes with RAS signaling.
• As a result, cancer cells receive fewer signals that promote growth.

Daraxonrasib is a targeted treatment, but that does not mean it is free of side effects.

• Stomatitis and other oral disorders
• Diarrhea

This is why the treatment needs to be given under medical supervision. Patients should not interpret the survival results as meaning that daraxonrasib is suitable for every person with pancreatic cancer.

The challenge is even greater in advanced disease. About 80% of patients are diagnosed after the cancer has already invaded nearby tissues or spread elsewhere.

That is why the results of RASolute 302 are important. In the trial, patients with metastatic pancreatic cancer lived a median of 13.2 months with daraxonrasib, compared with 6.7 months with chemotherapy.

The drug also kept the disease from worsening for longer and produced tumor shrinkage in more patients.

The approval is also important because RAS has been difficult to block with drugs for decades. Daraxonrasib provides a new way of interfering with this cancer-driving pathway and could open the door to treating other cancers involving RAS alterations.
