How does advanced cervical cancer increase lung metastases risk? A global study explains who faces the greatest risk.

Lung Metastases Affect Around 1 in 23 Cervical Cancer Patients
For the study, researchers conducted a systematic review and meta-analysis of published research examining the occurrence of lung metastases in women with cervical cancer. Using a random-effects statistical model, they pooled data from multiple studies to estimate the global burden of pulmonary spread.
The analysis found an overall lung metastases incidence of 4.31% (95% confidence interval: 3.25–5.37). While this percentage may appear relatively small, it represents thousands of women worldwide each year who develop cancer beyond the cervix.
The researchers also observed notable regional differences. Africa reported the highest estimated incidence at 7.42%, suggesting that geographic disparities in diagnosis, access to care, and treatment may influence disease progression.

Advanced Cervical Cancer Significantly Raises Lung Metastases Risk
One of the clearest findings from the analysis was the strong relationship between cancer stage and the likelihood of lung metastases.
Women diagnosed with Stage IV cervical cancer had an estimated 15.75% incidence of lung metastases, compared with just 3.12% among patients diagnosed with Stage I disease.
The results reinforce what oncologists have long observed in clinical practice: as cervical cancer advances, the likelihood of cancer cells spreading to distant organs rises substantially. Besides the lungs, the liver and bones remain other common sites of metastasis.

Aggressive Tumor Types Face Greater Risk of Lung Spread
Not all cervical cancers behave the same way. Researchers found that neuroendocrine carcinoma, a rare but aggressive subtype of cervical cancer, carried the highest risk of lung metastases.
Nearly 16.91% of patients with this histological subtype experienced pulmonary spread—far exceeding rates reported for the more common adenocarcinoma and squamous cell carcinoma. The findings suggest that tumor biology should be considered alongside disease stage when assessing a patient’s metastatic risk and planning follow-up care.
Survival After Lung Metastases Remains Limited
The study also evaluated outcomes after lung metastases develop. Researchers reported an average post-metastasis survival of approximately 32.6 months, although survival varied considerably across studies.
The wide range reflects differences in tumor subtype, extent of disease, treatment approaches, and patient characteristics. While advances in systemic therapies have improved outcomes for some patients, metastatic cervical cancer remains challenging to treat, making early identification of high-risk individuals especially important.
Improving Surveillance for High-Risk Cervical Cancer Patients
The researchers believe their findings could help clinicians refine risk assessment and monitoring strategies for cervical cancer patients.
Women with advanced-stage disease or aggressive tumor types such as neuroendocrine carcinoma may benefit from closer surveillance for distant spread, including the lungs. Earlier detection of metastases could enable more timely treatment decisions and improve care planning.
The authors also emphasize the need for continued research into strategies that can prevent, detect, and manage lung metastases more effectively, particularly in regions where the burden appears to be highest.
