AI vs doctors is gaining attention as simulated video consultations reveal how artificial intelligence performs in clinical reasoning, diagnosis, and patient care.

Can AI Outperform Doctors? A Striking New Test

Artificial intelligence is rapidly moving into the doctor’s office, but how well can it handle real-world medical conversations? A new 2026 study put AI head-to-head with primary-care physicians in simulated video consultations, producing striking results. (1✔ ✔Trusted Source

The study, which examined 100 clinical scenarios, found that AI performed on par with or better than doctors in several aspects of the consultations.

The findings are raising fresh questions about how AI could shape the future of healthcare.

AI Goes Head-to-Head With Doctors in Video Consultations

A 2026 study tested AMIE, a medical AI system, against 30 primary-care physicians using 100 clinical scenarios. (1✔ ✔Trusted Source

The scenarios covered cardiology, pulmonology, obstetrics/gynaecology, urology, gastroenterology, and neurology and musculoskeletal conditions.

Researchers designed cases to reflect real-world complexity, including multimorbidity and inconsistent patient information.

Clinical evaluators rated AI on par with or better than physicians in several areas, including history-taking, diagnosis, management, and physical observation and examination.

However, the results were not a complete win for AI: patient actors preferred doctors for rapport and partnership building, highlighting the importance of human connection in healthcare.

The study also involved simulated patients rather than people receiving actual medical care.

AI Shows Strong Clinical Reasoning in Medical Tests

The growing performance of AI clinical reasoning is also reflected in Stanford’s 2026 AI Index.

A multi-agent AI system achieved 85.5% on complex published medical case studies, compared with 20% for unaided physicians in the reported evaluation. (2✔ ✔Trusted Source

However, this comparison should not be interpreted as proof that AI is generally better than doctors.

Case-based evaluations and simulated consultations are different from treating patients in routine clinical practice, where safety, physical examination, uncertainty, and clinical responsibility remain major challenges.

    AI Is Rapidly Entering Healthcare

AI is already moving beyond experimental research into practical healthcare applications.

According to Stanford’s 2026 AI Index, the FDA authorized 258 AI-enabled medical devices in 2025.

The report also notes that AI tools generating clinical notes were associated with physicians spending up to 83% less time writing notes across multiple hospital systems. (2✔ ✔Trusted Source

These developments show why AI-powered healthcare, digital health technology, and AI medical consultation are becoming increasingly important areas of medical research.

    Can AI Replace Doctors?

The latest findings suggest that the more realistic future may be AI augmenting doctors rather than replacing them.

The video-consultation study found important strengths for AI in clinical reasoning and assessment, but doctors remained stronger in areas involving rapport and partnership with patients.

Researchers also identified limitations involving fine anatomical precision, subtle emotional cues, and high-frequency movements.

As AI moves closer to real-world patient care, future research will need to determine whether impressive performance in simulated consultations translates into safe, reliable outcomes in actual clinical settings.

How Audio-Visual AI Could Transform Medical Consultations

Audio-visual AI could make telehealth and AI medical consultations more like face-to-face visits.

Instead of relying only on written messages, AI can analyze a patient’s voice, facial expressions, movements, and other visual cues during a consultation.

But the technology still has challenges. AI needs to understand these cues accurately, respond quickly, and adapt to different patients and medical conditions.

It also needs strong clinical reasoning to turn what it sees and hears into useful medical guidance.

Researchers say more work is needed to determine whether AI’s performance in simulated consultations can translate into safe and effective care for real patients.

“Two heads are better than one.” As AI continues to evolve, its greatest potential may lie not in replacing doctors but in helping them make better-informed decisions.
