In the JAMA Perspective, Ezekiel J. Emanuel and colleagues argued that AI systems could eventually become better than physicians and physician-AI teams at several fundamental cognitive tasks. These include gathering patient information, diagnosing conditions, selecting tests, recommending treatments and managing chronic diseases.
The authors point to the rapid improvement of large language models and argue that the assumption that a doctor must always remain between an AI system and the patient should be reconsidered as the technology improves.
Their argument is not that doctors would disappear entirely. Physical examinations, surgery, deliveries and other hands-on procedures would still require human involvement. Instead, the authors suggest that autonomous AI could eventually take responsibility for some cognitive aspects of medical care.

The distinction is important. An AI system may be able to process medical information quickly or identify patterns across thousands of cases, but treating a patient involves decisions that may extend beyond the information contained in a medical record.
Doctors Are Becoming More Comfortable With AI
Despite the debate about AI replacing physicians, doctors’ use of the technology continues to expand.
The AMA’s 2026 survey found that more than three-quarters of physicians believe AI improves their ability to care for patients, compared with 65% in 2023. Physicians identified potential improvements in diagnostic accuracy and work efficiency as major advantages.
AI is also being viewed as a possible way to reduce administrative pressure. Seventy percent of physicians surveyed said they see AI as a tool that could automate tasks contributing to work-related burnout. At the same time, 88% expressed concern about potential skill loss, particularly among physicians who have been in practice for 10 years or less.
This creates a complicated picture: doctors see value in AI, but they also want safeguards around how it is introduced.
Privacy, Safety and Accountability Remain Concerns
Greater adoption does not mean physicians are ready to accept every AI tool.
The AMA survey found that

- Most physicians also want to be involved in decisions about which AI systems their organizations adopt.

What Would an AI-Doctor Partnership Look Like?
The hybrid model remains the more familiar approach. Under this model, AI handles information-heavy or repetitive work while doctors retain responsibility for clinical decisions.
There are clear potential benefits. AI can review large volumes of information quickly, help physicians keep up with medical literature and reduce documentation demands. That could allow doctors to spend more time speaking with patients and dealing with complex cases.
The AMA-DiMe framework describes a similar future in which AI supports new models of care while physicians continue to lead clinical practice and remain responsible for judgment and accountability.
But the authors question whether this arrangement should remain the default if autonomous AI becomes demonstrably better than physicians at certain cognitive tasks. Their argument suggests that healthcare may eventually need to compare three models directly: physician-only care, physician-AI collaboration and autonomous AI.
What Does This Mean for Patients?
For patients, the immediate change is likely to be increased exposure to AI rather than the disappearance of doctors. The latest debate suggests that the healthcare system may eventually have to rethink how those responsibilities are divided.
For now, the evidence points to a period of transition rather than a replacement of physicians. AI adoption among doctors is accelerating, but so is the discussion about safety, accountability and the limits of automation. The question facing healthcare is no longer whether AI will become part of medicine. It is how far its role should extend—and where human judgment must remain.
