Don’t Ignore Severe Period Pain, It Could Signal Endometriosis

Persistent menstrual pain should not be normalized, as earlier recognition of endometriosis symptoms may support more timely diagnosis and care.

Don`t Ignore Severe Period Pain, It Could Signal Endometriosis
    Highlights:

    • Severe period pain should never be dismissed as a normal part of menstruation
    • Recognizing endometriosis symptoms early may help reduce delays in diagnosis
    • Timely evaluation of painful periods can support earlier endometriosis diagnosis

Many women grow up believing that severe menstrual pain is simply part of life. However, recent evidence published in Frontiers in Medicine suggests that normalizing persistent, severe period pain can contribute to delayed diagnosis of endometriosis, allowing symptoms to continue for years before appropriate care begins.
Another systematic review in BJOG: An International Journal of Obstetrics & Gynaecology review found that women in the included studies waited between 5 and 12 years, on average across individual studies, to receive an endometriosis diagnosis () .

Delays varied widely depending on where people lived and how diagnosis was measured. The analysis also showed that delayed diagnosis is influenced by patient-related factors such as:

    • Postponing medical consultation
    • Provider-related factors including symptom dismissal or misdiagnosis
    • Healthcare system barriers such as referral pathways.

Together, these delays were linked with worsening symptoms, reduced quality of life, and poorer reproductive health outcomes. “Diagnostic delay is still present, primarily driven by physicians,” write Pauline De Corte and colleagues in the BJOG review.

Severe Period Pain and Endometriosis Symptoms Should Not Be Ignored

Endometriosis is a chronic condition in which tissue similar to the lining of the uterus grows outside the uterus. It can cause severe menstrual cramps, chronic pelvic pain, pain during intercourse, painful bowel movements or urination during menstruation, fatigue, and fertility challenges that can affect a woman’s quality of life.

According to the World Health Organization (WHO):

    • 190 million women and girls worldwide are living with endometriosis ().
    • 1 in 10 women of reproductive age is affected by the condition.
    • Endometriosis is one of the leading causes of chronic pelvic pain and infertility.
    • 35–50% of women with chronic pelvic pain or infertility are estimated to have endometriosis.

A college student who repeatedly misses classes every month because of unbearable cramps may assume she simply has a low pain threshold.

A working professional who schedules important meetings around her menstrual cycle may also believe this disruption is something she must live with. These experiences may deserve medical attention rather than silent acceptance.

Delayed Endometriosis Diagnosis Often Begins With Symptom Normalization

The analysis identified several reasons why diagnosis may be delayed:

    • Menstrual pain being accepted as normal
    • Delaying medical consultation despite persistent symptoms
    • Symptoms being mistaken for other conditions
    • Limited awareness among healthcare providers
    • Delays in referral to specialists

Delayed diagnosis is rarely caused by a single factor. Instead, it often reflects a combination of personal beliefs, healthcare experiences, and system-level barriers that can gradually extend the time before appropriate care is received ().

Why Early Recognition of Endometriosis Symptoms Matters

A delayed diagnosis can mean living with ongoing pain for longer than necessary. It may also affect emotional well-being, daily activities, relationships, work productivity, and reproductive health before the underlying condition is identified.

Someone who keeps a regular supply of painkillers in every handbag or routinely declines family outings during menstruation may see these adjustments as normal. Persistent symptoms that interfere with everyday life deserve a conversation with a healthcare professional.

Endometriosis Awareness Can Support Earlier Medical Care

The reviews highlight that greater awareness among both the public and healthcare professionals could help reduce diagnostic delays. Improving recognition of symptoms, encouraging timely medical consultation, and strengthening referral pathways may help more women receive appropriate evaluation sooner.

Evidence from both reviews points in the same direction. Severe period pain that consistently disrupts education, work, or daily routines should not automatically be accepted as a normal part of menstruation, especially when symptoms persist over time.

If severe menstrual pain is affecting your daily life, know that your symptoms deserve to be heard and evaluated. Seeking medical advice early may be an important step toward understanding your body and finding appropriate care.
